Pathophysiology of acne

Acne has been estimated to affect over 90% of the human population at some point in their lives. The pathophysiology of acne is multifactorial:

  1. Hormonally induced, excessive production of sebum
  2. Abnormal keratinisation of the lining of the pilosebaceous unit
  3. An overgrowth of bacteria (P. acnes)
  4. Rupture of the follicle and consequent inflammation.


How does IPL treat ACNE?

Dr Georghy’s revolutionary technique uses no gel or cooling device. The sparkling, pulsed light is delivered from a distance and through special crystals. This allows its rejuvenating energy to reach the skin, which Dr Georghy claims promotes remedial biological effects. The doctor is the first in the world to use this concept, and his reasoning is simple. “If there were no sun, there would be no life on earth," he says. "All living organisms and plants get light energy from a distance. Why should I not deliver light energy in a similar manner to the sun? All I did was copy Mother Nature." He calls his technique 'Distant IPL' or 'Sydney technique’. The entire treatment as conducted by Dr Georghy lasts approximately 30 to 40 minutes and is a combination of several light therapies. You may feel a slight sting, like the snapping of a small rubber band. An anaesthetic cream may be used, but is seldom required. Most patients report no pain and there is no downtime. Some 30 per cent of Dr Georghy's patients report mild redness that disappears. He has been treating acne since 1993.


Clinical Results

The clinical observation over the last several years resulted in a 100% response rate. A reduction of new acne of over 70-80% was seen after 3 to 6 weeks and maintenance treatment was found to decrease the frequency and the severity of outbreaks over 75%. Cure was achieved after prolonged maintenance therapy particularly in patients where conventional therapy has failed.


Adverse Effects

With the exception of transient redness, lasting 15 to 30 minutes, no adverse effects were observed. Patients were able to apply make up and return to normal activities immediately with no downtime (lunch-time treatment).


Available treatments

Multiple treatments are available including topical therapy in the form of cleansers, creams, gels, lotions and masks. In addition, oral antibiotics and oral Isotretinoin (Roaccutane) are also used in the treatment of moderate to severe acne. Many side effects are associated with traditional therapies including skin irritation, erythema, cheilitis, dry eyes, liver and renal abnormalities, anaemia, leukopenia, lupus erythematosus, brain tumour (pseudotumor cerebri), thyroid abnormalities, hypersensitivity reactions, fatal defects, diffuse skeletal hyperostosis, photosensitivity, irreversible night blindness, depression, hypertriglyceridemia and hypercholesterolemia.


Summary

PhotoLaser therapy is the most revolutionary medical and cosmetic treatment since the discovery of penicillin. We are only at the beginning of our understanding of the potential of this mighty therapy. He believes that in future there could be heliotherapeutic clinics where the sun energy would come through interchangeable ceiling mounted filters, delivering ‘Mother Nature Therapy’. There could be Photorejuvenation studios (similar to tanning studios) that people can attend to receive photorejuvenation treatment.
